Larval substrate: Unknown
Phenology: Adult: 7/09 to 7/26 (2 records)

Comment: A characteristic species with three black dots, believed to be this species (matches well the original description and illustration), although similar-looking individuals in BOLD are placed under Calosima lucidella, a pure white species described from Florida. C. melanostriatella was described from PA and CT.
